| Appointment： | Only those who has consulted a doctor in our clinic on the previous day are eligible.

| Things to bring： | Box lunch（bring milk powder and bottle for the formula-fed infant）
Snacks, cup, and chopsticks etc. Medicine（with the name of the medicine and the information on the medicine） Hand towel, bath towel, diapers, and a plastic bag for dirty stuffs. Change of clothes (T-shirt, trousers, and pants etc). Maternity passbook, health insurance card & infant medical certificate |

＊ Write your kid’ name in the things you will bring to day care.
See a doctor at 8:30 on the day.

Bring the used diapers home.
Diapers provided by our care room is 50Yen a piece.
If your pick-up will be late, please inform us.
We may not able to take care of your child if the quotas are full.
